Cataract surgery via phacoemulsification with intraocular lens (IOL) placement in the capsular bag is the gold standard in the presence of adequate capsular support. However, when capsule and/or zonular fibers are weak or absent, alternate fixation strategies are required. Common alternative options include retropupillary iris-claw IOLs (RP-IC IOLs) and scleral-fixated IOLs (SF IOLs). In the present review of 87 articles with 2174 eyes implanted with RP-IC IOLs and 2980 eyes with SF IOLs, we discuss the published literature with respect to safety and efficacy. Although the studies reporting outcomes of these IOLs have been performed in patients with different concomitant conditions, visual and refractive outcomes were found to be comparable between RP-IC IOLs and SF IOLs. RP-IC IOL implantation seemed to provide equivalent or a potentially lower rate of complications than SF IOL implantation. Data from the literature also suggest that the surgical technique of RP-IC IOL implantation is relatively simpler with correspondingly shorter surgical times.

This study aimed to summarize the outcomes reported when swept-source optical coherence tomography (SS-OCT) is used for ocular biometry. A literature search was performed to identify publications reporting clinical outcomes of patients measured with commercial SS-OCT. Twenty-nine studies were included in this review. A comprehensive analysis of the available data was performed, focusing on parameters used for intraocular lens (IOL) power calculation in cataract surgery, including keratometry, central corneal thickness, white-to-white distance, anterior chamber depth, lens thickness, axial length, IOL power, and pupil diameter. Different metrics for repeatability, reproducibility, and agreement between devices were analyzed. In general, SS-OCT biometers provide excellent repeatability and reproducibility outcomes; however, the differences obtained for some parameters measured in agreement studies should be carefully analyzed to validate the interchangeability between devices. The good outcomes reported lead us to conclude that optical biometers based on SS-OCT technology are likely to become the gold standard for ocular biometry.

Presented is a modified technique for removing a failed endothelial allograft in an aniridic aphakic eye that reduces the risk of posterior dislocation. This technique involves the creation of an artificial iris plane using a Sheets glide by trimming it into the shape of a dagger so that it can be pulled through the anterior chamber and fixated by both its passage through a paracentesis incision and the main incision. The Sheets glide then acts as a physical barrier, or scaffold, to allow safe removal of the failed endothelial allograft and subsequent repeat endothelial keratoplasty while avoiding posterior dislocation of the tissue into the vitreous cavity.

A 21-year-old man was referred for refractive surgery evaluation. He had a history of advanced keratoconus that led to penetrating keratoplasty (PKP) in the right eye 2 years ago. Spectacle dependence and poor visual quality in this right eye is his chief concern. He cannot tolerate contact lenses. The uncorrected (UDVA) and corrected distance visual acuity (CDVA) were 20/400 and 20/50, respectively. The manifest refraction was ?2.75 ?8.00 × 165 in the right eye.
