Intermittent Explosive Disorder Screening Questionnaire (IED-SQ)
Instructions: Most everyone experiences aggressive thoughts or behaviors at different points in life. Please answer the following questions about your experiences regarding this as accurately as possible. Your responses will remain completely confidential.
Part 1:
How many times would you say you have done the following things over the course of your life, since you turned age 13?
0 = Never happened (eg, zero)
1 = Only happened "once" (eg, one)
2 = Happened "a couple" or "a few" times (eg, two to three)
3 = Happened "several" times (eg, four to nine)
4 = Happened "many" times (eg, 10 or more)
5 = Happened "so many" times that I couldn't give a number

Here, we are asking for a bit more detail on your response in Part 1.

5b. Has there been a time in your life when you hit individuals and/or "broke things" in anger a total of three or more times in a year when you were not taking any medication, drinking alcohol, or experiencing the effects (high or withdrawal) of any drug?   Yes   No

Part 1
  • Lifetime aggression positive: ≥12 on Part 1 items
Part 2
  • A1 positive: ≥3 months on question 1a.
  • A2 positive: ≥3 times on question 2.
  • C positive: "Unplanned" on question 3.
  • D positive: "Yes" on question 4a or 4b.
  • E positive: ≥6 years of age on question 1b or 2a.
  • F positive: "Yes" on question 5a or 5b.
IED-5 positive if:
  • Lifetime aggression positive and
  • A1 or A2 positive and
  • C positive and
  • D positive and
  • E positive and
  • F positive
IED-5 current if:
  • "Yes" on question 1C and/or 2b
