Diverting loop ileostomy/colonic lavage
(A) A loop diverting ileostomy is created 20 to 40 cm from the ileocecal valve. A 26 to 30 French urinary catheter is inserted into the efferent limb of the ileostomy and the balloon is inflated to 10 cc below the fascia. Eight liters of warm polyethylene glycol (PEG) solution are instilled into the colon and collected by a rectal tube.
(B) Postoperatively, the urinary catheter is replaced with a Malecot-type catheter. Vancomycin flushes (500 mg in 500 mL of Lactated Ringer solution) are instilled three times a day for 7 to 10 days and collected by the same rectal tube.
