Targeting the interleukin-5 pathway in EGPA: evidence, uncertainties and opportunities

Alvise Berti, Fabiola Atzeni, Lorenzo Dagna, Stefano Del Giacco, Giacomo Emmi, Carlo Salvarani, Augusto Vaglio

doi : 10.1136/ard-2022-223044

Only a minority of patients with eosinophilic granulomatosis with polyangiitis (EGPA) can be weaned-off glucocorticoids (GC) using conventional treatment strategies. The development of biological agents specifically inhibiting the IL-5 pathway provided the opportunity to treat EGPA by targeting one of the crucial regulators of eosinophils, reducing the GC dose required to control the disease.

Clinical trials in systemic lupus erythematosus: the dilemma—Why have phase III trials failed to confirm the promising results of phase II trials?

Ana Lorenzo-Vizcaya, David Alan Isenberg

doi : 10.1136/ard-2022-222839

Systemic lupus erythematosus (SLE) is an autoimmune rheumatic disease of unknown aetiology, characterised by the production of auto-antibodies and formation of immune complexes against self-antigens and complement activation. This inflammatory response can lead to tissue infiltration and eventually, to organ damage.

doi : 10.1136/ard-2022-222835

Objectives The expanded therapeutic arsenal in rheumatoid arthritis (RA) raises new clinical questions. The objective of this study is to compare the effectiveness of cycling Janus kinase inhibitors (JAKi) with switching to biologic disease-modifying antirheumatic drug (bDMARD) in patients with RA after failure to the first JAKi.

3D genome alterations in T cells associated with disease activity of systemic lupus erythematosus

Ming Zhao, Delong Feng, Longyuan Hu, Lin Liu, Jiali Wu, Zhi Hu, Haojun Long, Qiqi Kuang, Lianlian Ouyang, Qianjin Lu

doi : 10.1136/ard-2022-222653

Objectives Three-dimensional (3D) genome alterations can dysregulate gene expression by rewiring physical interactions within chromosomes in a tissue-specific or cell-specific manner and lead to diseases. We aimed to elucidate the 3D genome structure and its role in gene expression networks dysregulated in systemic lupus erythematosus (SLE).

doi : 10.1136/ard-2022-222935

Background Idiopathic inflammatory myopathies (IIM) are a group of autoimmune diseases characterised by myositis-related autoantibodies plus infiltration of leucocytes into muscles and/or the skin, leading to the destruction of blood vessels and muscle fibres, chronic weakness and fatigue. While complement-mediated destruction of capillary endothelia is implicated in paediatric and adult dermatomyositis, the complex diversity of complement C4 in IIM pathology was unknown.

Targeting FoxO transcription factors with HDAC inhibitors for the treatment of osteoarthritis

Hiroki Ohzono, Yiwen Hu, Keita Nagira, Haruhisa Kanaya, Naoki Okubo, Merissa Olmer, Masafumi Gotoh, Ichiro Kurakazu, Yukio Akasaki, Manabu Kawata, Emily Chen, Alan C Chu, Kristen A Johnson, Martin K Lotz

doi : 10.1136/ard-2021-221269

Objectives Osteoarthritis (OA) features ageing-related defects in cellular homeostasis mechanisms in articular cartilage. These defects are associated with suppression of forkhead box O (FoxO) transcription factors. FoxO1 or FoxO3 deficient mice show early onset OA while FoxO1 protects against oxidative stress in chondrocytes and promotes expression of autophagy genes and the essential joint lubricant proteoglycan 4 (PRG4). The objective of this study was to identify small molecules that can increase FoxO1 expression.

Synovial fibroblasts assume distinct functional identities and secrete R-spondin 2 in osteoarthritis

Alexander J Knights, Easton C Farrell, Olivia M Ellis, Lindsey Lammlin, Lucas M Junginger, Phillip M Rzeczycki, Rachel F Bergman, Rida Pervez, Monique Cruz, Eleanor Knight, Dennis Farmer, Alexa A Samani, Chia-Lung Wu, Kurt D Hankenson, Tristan Maerz

doi : 10.1136/ard-2022-222773

Objectives Synovium is acutely affected following joint trauma and contributes to post-traumatic osteoarthritis (PTOA) progression. Little is known about discrete cell types and molecular mechanisms in PTOA synovium. We aimed to describe synovial cell populations and their dynamics in PTOA, with a focus on fibroblasts. We also sought to define mechanisms of synovial Wnt/β-catenin signalling, given its emerging importance in arthritis.
