Superficial fungal infections which effect the skin, hair, and nails, are widely observed in the world. Causative agents are Candida species, dermatophytes, and Malassezia species. In recent years, new antifungal agents and formulations have been developed for these infections. In addition, laser therapies have been tried for onychomycosis. There are also studies on the antifungal activity of some herbs and propolis. In the present study, relatively new treatment methods of superficial fungal infections will be described.

Ultraviolet (UV) irradiation to the eye induces photoimmunosuppression. In here, we examined the effect of green odor against immunosuppression of contact hypersensitivity in the eye induced by ultraviolet B (UVB) irradiation. Ectropion is a rather frequent complication of ichthyosis with negative functional and esthetic impact. Lid surgery can provide significant improvement. Treatment of eosinophilic dermatosis of hematologic malignancy (EDHM) has been challenging with a minimal response to several modalities. Only systemic corticosteroids have consistently proven successful in suppressing the rash; however, the response is often only partial, and symptoms tend to recur as the dosage is tapered. This demonstrates the need for effective maintenance therapy. Herein, we present a case of EDHM that, following a 1 month prednisone taper and brief trial of narrow?band ultraviolet B phototherapy, has remained well?controlled with omalizumab. This suggests that omalizumab may represent an effective therapeutic alternative in EDHM management with low long?term toxicity.

Interstitial granulomatous dermatitis (IGD) is a rare dermatosis generally seen in the setting of rheumatic diseases, but also hematological disorders, internal malignances, infections, or drug induced. Herein, we report an exceptional case of an IGD with a clear chronological association with tocilizumab onset and cessation in a patient with adult?onset Still's disease. We review the granulomatous cutaneous reactions so far reported with this novel therapy: sarcoidosis, granuloma annulare, and IGD. Tocilizumab is a humanized anti?interleukin 6 receptor monoclonal antibody useful for the treatment of various systemic inflammatory disorders. Lately, it has found useful also for granulomatous diseases such as giant cell arteritis and even a promising response in IGD. Therefore, we believe our case adds the possibility of an IGD presenting as a paradoxical reaction.

We report 16 patients of tinea cruris who presented with an asymptomatic red scrotum due to fixed dose combination creams containing antifungal agents and topical steroids applied to the thighs as a treatment of tinea cruris. The erythema resolved upon starting appropriate treatment with single molecule antifungal creams and oral antifungal drugs in about six weeks' time in majority of cases. We propose that this clinical presentation may be akin to red scrotum syndrome (RSS). Due to its asymptomatic nature in most of the cases, we propose that this could be a forme fruste of RSS.

We report three cases of severe hypertrichosis in healthy infants, who did not present any type of endocrinological pathology and whose parents used topical minoxidil for the treatment of their baldness. Any type of direct application or administration of the product was ruled out. Hypertrichosis is considered to have occurred through skin?to?skin contact with the parent, and even through fomites. In 2017, the Food and Drug Administration approved dupilumab for patients with moderate to severe atopic dermatitis (AD) refractory to topical therapies; however, clinical trials specifically excluded patients with human immunodeficiency virus (HIV). Here, we describe the effective and uncomplicated treatment of AD with dupilumab over a 23?month period in a patient with HIV. Throughout the treatment duration, the patient demonstrated marked improvement in AD severity, while maintaining stable CD4 T cell counts and viral load. Bromoderma is a rare hypersensitivity reaction caused by bromide intake. It was relatively frequent in the early years of the previous century because of the common use of bromide?containing solutions in pediatrics due to their antispasmodic, expectorant, sedative, and anticonvulsant effects. Although recently prohibited in many countries, bromides are still used as an adjuvant anticonvulsant drug and still present in some over the counter antispasmodics and analgesics. Bromoderma usually present with pustular and vegetating lesions that may represent a diagnostic challenge for dermatologists. We describe a severe case of vegetating bromoderma that showed an excellent response to the withdrawal of the bromide?containing medication associated with systemic steroid administration.

Drug?induced sarcoidosis has been rarely described but it constitutes a potential side effect of immunomodulatory medications. We report a case of rituximab?induced scar sarcoidosis and review similar published cases. Although there is evidence of B?cell dysregulation in the pathogenesis of sarcoidosis, the use of rituximab for this disease needs to be carefully evaluated based on reports of worsening and de novo development of sarcoidosis after rituximab therapy.

An atypical cellular blue nevus, a benign mass, may sometimes transform into a malignant melanoma. Here, we report a rare case of melanoma arising in a large congenital vulvar blue nevus. A 28?year?old Chinese woman presented to our hospital with a chief complaint of a vulvar mass that had persisted for 8?years. The patient underwent extensive local excision, followed by reconstructive surgery of the female reproductive tract. The mass was diagnosed as being a vulvar malignant melanoma. Postoperatively, the patient received interferon immunotherapy and recovered without complications. No evidence of recurrence was observed after 32?months of follow?up. Our case thus shows that comprehensive treatment with surgery supplemented by immunotherapy can be effective against a malignant melanoma arising in a vulvar blue nevus.

A 65?year?old women presented with sharply demarcated macules and small nodules on her face and neck for 2.5?years. They first appeared on her left face and gradually spread to her whole face and neck. The lesions were slightly pruritic. Her history was remarkable only for a high dose of topical tretinoin cream 0.025% dosage. On the basis of distribution and history, we made the diagnosis of contact dermatitis for the macules. A biopsy of a red papule from her neck revealed characteristics of pyogenic granuloma. However, white patches appear on his face at the third month follow?up after suspension of the tretinoin cream; Wood's lamp examination was consistent with vitiligo. It is the first report of a dose?response variant of this adverse reaction (Contact dermatitis combined with Pyogenic granulomas).

Immune checkpoint blockades were reported to result in clinical responses in inoperable and metastatic cutaneous squamous cell carcinoma (cSCC). This report describes an 87?year?old woman with recurrent cSCC that was initially responsive to cetuximab (the monoclonal antibody against epithelial growth factor receptor) but eventually became refractory to cetuximab and multiple subsequent salvage chemotherapy regimens.

Intravenous immunoglobulin (IVIg) is increasingly used for the treatment of inflammatory and autoimmune diseases. Although skin reactions to IVIg therapy are usually minor, rare, and not life?threatening, dermatologists need to recognize the nature of these adverse reactions. We describe a 33?year?old man suffering from demyelinating polyneuropathy who developed dyshidrotic eczema on the palms and flaky grayish?white scales on an erythematous base on his face after the administration of IVIg.

Angiokeratomas are benign vascular neoplasms that arise as solitary or multiple lesions, most commonly treated with excision, electrodessication, cryotherapy, or laser therapies. This case presents a young female whose solitary angiokeratoma was treated with topical 1% sirolimus cream, improving the appearance, symptoms, and size of the lesion. Topical sirolimus cream may be a noninvasive treatment option for angiokeratomas with fewer risks than standard therapy that may be feasible and preferable for some patients.

The authors aim to present the butterfly effect, a concept based on the theory that small changes might have a powerful effect, as an example of the important connection between diet and acne. Western diet is currently a well?known environmental factor which, mainly via the overstimulation of mTORC1 (mammalian target of rapamycin complex 1), is responsible for the development and aggravation of acne and other age?related diseases of civilization. From the authors' point of view, “the butterfly effect” extrapolated to acne and diet depicts the importance of dietary interventions in acne so as to prevent more serious mTORC1?driven diseases of civilization like obesity, diabetes, and cancer.

Raynaud's phenomenon is a vasospastic disorder clinically characterized by cold or stress?induced discoloration of the skin, pain and ulcers of the fingers or toes. Although this phenomenon might be self?limiting, there is a subgroup of patients requiring a therapeutic approach. The majority of patients do well on conservative measures; however, there is also a subgroup requiring systemic treatment. The efficacy of these systemic treatments is currently not thoroughly investigated. Furthermore, no uniform guidelines exist regarding the choice for a treatment option. In the past several years, several reports have shown the benefits of botulinum toxin for the treatment of Raynaud's phenomenon. In this case series, we report our experience with botulinum toxin type A in the treatment of Raynaud's phenomenon.

Mycobacterium caprae, a member of the Mycobacterium tuberculosis complex, causes tuberculosis in humans and animals, but the incidence in humans is very low worldwide. Direct contact with animals and with unpasteurized dairy products is the most frequent source of contagion. Cutaneous tuberculosis supposes a diagnostic challenge due to its variety of clinical manifestations and the difficulty in demonstrating resistant acid?alcohol bacilli in histological samples. In cases of high suspicion, the initiation of antituberculosis treatment should be considered empirically. Combined therapeutic regimens are necessary to prevent long?term resistance and complications, although the duration of treatment has not been fully established.

The objective of our study was to determine thiol/disulphide homeostasis in recurrent aphtous stomatitis (RAS) patients. A prospective monocentric study was designed. Forty?three recurrent aphtous stomatitis patients and 43 healthy subjects were included to study. Venous blood samples collected and assessed with novel automatic system. Results compared statistically. Disulphide levels were found significantly higher in RAS patients than control group. There was no significant difference between native thiol and total thiol levels. Thiol/disulphide homeostasis is impaired in RAS patients in favor of disulphide levels compared with control group. To the best of our knowledge, the present study is the first examination on the correlation between thiol and disulfide homeostasis in patients with RAS.

During COVID?19 outbreak hospitals were congested and infliximab was interrupted. Thus, we performed this observational study to understand the consequent burden of complications in these special cluster of psoriatic patients. We followed up 56 psoriatic patients who were receiving Infliximab treatment by telephone. The majority of patients had lesions exacerbation, accompanied by anxiety emotion. It is suggested that reserving common drugs for psoriasis at home is necessary. Besides, telemedicine should be advocated as a main medical visit mode during the outbreak of COVID?19.

Superficial chemical peeling using Modified Jessner's solution has been used for the treatment of facial pigmentation but repetitive chemical peeling can cause irritation and post?inflammatory hyperpigmentation. In this series, we evaluated seven patients who were treated with a combination of 1064?nm Q?switched Nd: YAG laser and Modified Jessner's peel for eight sessions with 85.7% patients showing more than 50% reduction in pigmentation. The added therapeutic benefit of the combination helped to achieve a significant reduction in pigmentation faster with a lesser number of sessions and reduced cost.

The subacute cutaneous lupus erythematosus (SCLE) is a distinct subtype of lupus erythematosus (LE) representing specific clinical and serological features. Almost 20%?30% of the cases with SCLE are predicted to associated with medications. Thiazide diuretics, terbinafine, antiepileptic, and proton pump inhibitors are the best?known drugs to induce drug?related SCLE. Herein we want to present a 65?year?old female with alendronate induced SCLE, resistant to classical therapies, and respond well to intravenous immunoglobulin (IVIG), suggesting that IVIG could be an alternative treatment in patients resistant to classical treatment protocols.

Risankizumab is a novel anti?interleukin 23 humanized antibody developed to treat severe psoriasis. In this paper, we analyze the key information about this new drug and the results of phase 3 clinical trials already present in literature in order measure its safety and effectiveness in every day practice. Risankizumab seems to be one of the best performing drugs in the treatment of psoriasis, with a good safety profile and a dosage regimen less frequent than all other biologic agents, although head?to?head randomized clinical trials with other anti?interleukin treatments will be necessary in order to confirm these preliminary results.

5?Aminolevulinate (ALA) patches with red light (630?nm light source and a total light dose of 37?J/cm2) is an effective treatment indicated by food and drug administration (FDA) and european medicines agency (EMA) only for grade I to II actinic keratosis located on the scalp and face. Currently, there are no efficacy data on their use in the treatment of other types of epithelial neoplasms. We analyzed the efficacy of ALA patches in seven superficial basal cell carcinomas (sBCCs) that occurred in four patients. All lesions were treated with topical ALA patches. A complete response of all sBCCs was achieved at week 24 after treatment. Our study suggests that ALA patches for sBCCs have good efficacy rates and excellent safety profile.

Intralesional steroid injection is a treatment method frequently used to resolve a large number of orthopedic, rheumatological, dermatological, and neurological disorders. Although this treatment is very effective, it is not without possible side effects, both systemic and local, among which we can mention pain, bleeding, ulceration, atrophy, pigmentary changes, calcification, secondary infections, formation of granulomas, allergic reactions and, in very rare cases, the development of linear atrophy, and hypopigmentation. Here, we present a case of frontal linear skin atrophy after intralesional steroid injection for the treatment of alopecia areata (AA) in a 29?year?old patient, successfully treated with a hyaluronic acid filler.
