High ankle sprains typically occur during high-intensity athletic activities such as competitive American football, soccer (football), and hockey. An external rotation force applied to a dorsiflexed ankle is the most common mechanism. This can happen when the leg is rotated forcefully against a planted foot or toe. Often, such injuries occur during contact with other athletes when the athlete is standing.
