Instructions to caregivers or primary care provider on completing the voiding/drinking diary:
Fill out this chart on a day without school or kindergarten (weekend or holiday). This should start on one morning and continue through to the next morning. If possible, fill out the diary 2 days in a row as this is even more reliable.
Talk to the child beforehand. They should tell you when they want to go to the toilet and should empty the urine into a measuring cup. Measure the amount of urine, and record it with the time of day. You do not have to collect the urine but can discard it afterward.
If the child needs to strain to start voiding or if the stream is interrupted, note this in the "Straining/interrupted stream" column.
If the child feels a sudden urge to go to the toilet, note this in the "Urge" column.
If the child wets their clothes, note the time and if the clothes were wet or damp.
If the child crosses their legs, squats, or tries to hold back the urine in any other way, note the time and observation in the "Comments/observations" column.
Note the time of each bowel moment, and any episode of fecal soiling, in the "Comments/observations" column.
Measure and note the amount of fluids and the time the child drinks during the day.
