Medication considerations in athletes with URTIs

Medication considerations in athletes with URTIs
Medication Risks and implications Our approach
Antibiotics
Fluoroquinolones Tendinopathy/tendon rupture Avoid if possible, stop if symptoms of tendon pain arise
Photosensitivity Emphasize use of sunscreen and protective clothing, particularly if participating in outdoor sports
QTc prolongation – May fail electrocardiogram (EKG) screening (if required for clearance for participation) Reasonable to avoid where possible if athlete will have electrocardiogram (EKG) checked for clearance/eligibility
Macrolides QTc prolongation – May fail EKG screening (if required for clearance for participation) Reasonable to avoid where possible if athlete will have electrocardiogram checked for clearance/eligibility
Tetracyclines/sulfonamides Photosensitivity Emphasize use of sunscreen and protective clothing, particularly if participating in outdoor sports
Any antibiotic Antibiotic-associated diarrhea – Decreased performance/increased risk for dehydration Use narrow-spectrum antibiotics, avoid extended courses
Fatigue (possible) – Impaired performance When possible, avoid dosing antibiotics (eg, prophylactic antibiotics for acne) around the time of competition
Medications used for symptom relief
Expectorants None May be used
Antihistamines Anticholinergic effects – Increased risk of dehydration/heat illness Advise to stay well-hydrated
Sedation – Increased risk of injury Avoid in competition
Antitussives Fatigue – Increased risk of injury Use with caution
Oral decongestants May contain banned substances Avoid in competition
Dehydration/hyperthermia Advise to stay well-hydrated
Antipyretics May mask a fever Should not be used to mask fever in order to allow participation in competition
