(A) The transhepatic tube with appropriate side holes is passed into the mucosal diverticulum and anchored to the Roux-en-Y loop.
(B) The sleeve of jejunal mucosa is carried up into the intrahepatic bile ducts by withdrawing on the transhepatic tube. The jejunal wall is then anchored to the liver capsule.
