If a standard push-up is included in a rehabilitation or strength program but is too difficult or causes pain in the shoulder or elbow, a patient may start with push-ups against a wall. As strength improves, the patient gradually transitions to performing the exercise on increasingly lower surfaces (eg, chair, step), until it can be performed on the floor.
