Paramedian neuraxial needle insertion

For the paramedian approach to neuraxial needle placement, the needle is inserted 10 to 15 degrees from midline, in a slightly cephalad direction. The aim is to puncture the dura in the midline. This approach is particularly useful for the patient with reduced space between adjacent spinous processes.
