Diagnostic criteria for FASD
FASD with sentinel facial features:
All of the following:
  • Prenatal alcohol exposure confirmed or unknown
  • All three sentinel facial featuresΔ
  • Meets criteria for CNS involvement:
    • Any age: Severe impairment in ≥3 neurodevelopmental domains§
    • For children ≤6 years: Microcephaly
FASD without sentinel facial features:
Both of the following:
  • Confirmed prenatal alcohol exposure
  • Severe impairment in ≥3 neurodevelopmental domains§
Special designation (not a diagnosis)
At risk for neurodevelopmental disorder and FASD, associated with prenatal alcohol exposure (whether or not child has all 3 sentinel facial featuresΔ):
All 3 of the following:
  • Confirmed prenatal alcohol exposure, and
  • Some indication of neurodevelopmental disorder, but does not meet CNS criteria as described above, and
  • Plausible explanation for lack of meeting CNS criteria (eg, incomplete evaluation, child is too young for adequate assessment)
FASD: fetal alcohol spectrum disorder; CNS: central nervous system.

¶ Via reliable clinical observation; self-report; reports by a reliable source; medical record documentation of positive blood alcohol concentrations; alcohol treatment; or other social, legal, or medical problems related to drinking alcohol during pregnancy. Estimated dose of alcohol consumption must be at a level known to be associated with neurodevelopmental effects.
Δ Palpebral fissure length <3rd percentile; philtrum rated 4 or 5 on University of Washington Lip-Philtrum Guide; upper lip rated 4 or 5 on University of Washington Lip-Philtrum Guide.
Global score or a major subdomain score on a standardized neurodevelopmental measure that is ≥2 standard deviations below the mean, with appropriate allowance for test error.
§ Motor skills; neuroanatomy/neurophysiology; cognition; language; academic achievement; memory; attention; executive function, including impulse control and hyperactivity; affect regulation; adaptive behavior, social skills, or social communication.
