Fiberoptic intubation via supraglottic airway: Step 1 - Initial ETT placement
If blind intubation via a supraglottic airway has been unsuccessful and fiberoptic intubation is to be performed, partially withdraw the ETT so the distal portion remains within the supraglottic airway.
