Indications for intravenous (IV) fluid administration following endurance race*
WARNING: Before giving any IV fluids, the clinician must be certain the runner or racer does not have acute hyponatremia (most likely from excessive hydration during race)
  1. Persistent orthostatic blood pressure drop or persistent symptoms of exercise-associated collapse (EAC) despite 20 to 30 minutes of supine rest with leg elevation and oral fluids
  1. Persistent temperature >104°F (40°C) or <95°F (35°C) despite appropriate, standard interventions to cool or warm the patient
  1. Hypoglycemia <60 mg/dL (3.3 mmol/L)
  1. Severe, unabating muscle cramps in legs or throughout body
  1. Persistent nausea and vomiting, and unable to tolerate oral fluids
  1. Significant diarrhea with ongoing fluid losses
  1. Confused or clouded sensorium
  1. "Not doing well" per judgment of experienced medical staff
* This is a list of some relatively common and important indications. It is not meant to be exhaustive and clinicians must use their judgment to determine whether placement of an IV catheter is necessary.
