(A) Chest radiograph shows a large right pneumothorax with complete collapse of the right lung (arrowhead) in a patient with traumatic rupture of the right hemidiaphragm; the image also shows shift of the trachea to the left and widening of the right intercostal spaces, when compared to the left side. Multiple gas-containing bowel loops project above the level of the right hemidiaphragm (dashed arrow).
(B) Prior to the development of the pneumothorax, computed tomography of the chest in the same patient shows multiple intrathoracic loops of bowel at the periphery of the ruptured right hemidiaphragm (arrow).
