  • Cortical destruction is typical ("moth-eaten" or permeative pattern)
  • Cortical destruction is usually absent
Extension
  • Extends into the soft tissue
  • Confined by natural barriers (eg, growth plate, cortex)
  • Does not extend into the soft tissue
* Aggressive nonmalignant bone lesions include aneurysmal bone cyst, chondroblastoma, chondromyxoid fibroma, and giant cell tumor.
¶ The absence of periosteal reaction does not exclude a malignant lesion.
