Panel A demonstrates a centrally directed, moderate mitral regurgitant jet. Panel B shows a severe, eccentric, anteriorly directed mitral regurgitant jet in the setting of a flail leaflet. The eccentric jet adheres to the wall of the LA (Coanda effect), and has a large proximal flow convergence zone (arrow). Since the jet area is smaller in eccentric jets compared to central jets, the use of an eccentric jet area for assessment of mitral regurgitation severity is not recommended.
