TSS: transsphenoidal surgery; ITT: insulin tolerance test (insulin-induced hypoglycemia test); ACTH: corticotropin. * Pituitary macroadenoma is defined as pituitary mass >1 cm. ¶ Hydrocortisone is typically given as a divided dose, 2 to 3 times per day, and occasionally as a single morning dose.
