* The onset of action of these drugs is often delayed. We often continue prednisone early in the course of therapy until clinical improvement. Prednisone is subsequently tapered as tolerated. Similarly, HCQ or MTX are frequently continued while awaiting a response. The best course for patients who fail these agents is unclear. A variety of other therapies have been utilized for refractory cutaneous sarcoidosis, but efficacy data are limited.
