81 to 160 Any 80 mg IV bolus followed by 10 mg/hour infusion 5 mg metolazone once daily
161 to 240 Any 80 mg IV bolus followed by 20 mg/hour infusion 5 mg metolazone twice daily
>240 Any 80 mg IV bolus followed by 30 mg/hour infusion 5 mg metolazone twice daily
Failure of the suggested dose to induce adequate diuresis should prompt escalation to the next level of dosing.
