The top ultrasound image (A) shows an intact syndesmosis. The isoechoic, fibrillar ligament (arrow) is clearly seen and extends completely from tibia to fibula. The bottom ultrasound image (B) shows a torn syndesmosis. Note the hypoechoic (dark) hemorrhage and edema (thick arrow) over the torn ligament (dashed arrow), the discontinuity between the fibula and tibia, and the ligament stump protruding from the tibia (arrowhead).
