ALERT: US Boxed Warning
Hepatotoxicity:

Pexidartinib can cause serious and potentially fatal liver injury, including vanishing bile duct syndrome. Monitor liver tests prior to initiation of pexidartinib and at specified intervals during treatment. Withhold and dose reduce or permanently discontinue pexidartinib based on severity of hepatotoxicity. Monitoring and prompt cessation of pexidartinib may not eliminate the risk of serious and potentially fatal liver injury.

Pexidartinib is available only through a restricted program called the TURALIO Risk Evaluation and Mitigation Strategy (REMS) Program.

Tenosynovial giant cell tumor, symptomatic

Tenosynovial giant cell tumor, symptomatic: Oral: 250 mg twice daily with a low-fat meal (~11 to 14 g total fat) until disease progression or unacceptable toxicity. Administration with a high-fat meal (~55 to 65 g total fat) increases pexidartinib concentrations and may increase the risk for adverse reactions, including hepatotoxicity.

Missed dose: If a dose is missed or vomited, administer the next dose at its scheduled time.

Dosage adjustment for concomitant therapy: Significant drug interactions exist, requiring dose/frequency adjustment or avoidance. Consult drug interactions database for more information.

Dosing: Kidney Impairment: Adult

Note: Kidney function estimated by the Cockcroft-Gault equation (using actual body weight).

CrCl 15 to 89 mL/minute: Reduce dose to 125 mg in the morning and 250 mg in the evening (total of 375 mg/day); administer with a low-fat meal.

Pexidartinib is not expected to be dialyzable (due to high plasma protein binding).

Dosing: Liver Impairment: Adult

Hepatic impairment prior to treatment initiation:

Mild impairment (total bilirubin ≤ ULN with AST > ULN or total bilirubin >1 to 1.5 times ULN and any AST): No dosage adjustment necessary.

Moderate impairment (total bilirubin >1.5 to 3 times ULN [not due to Gilbert syndrome] and any AST): Reduce dose to 125 mg twice daily with a low-fat meal.

Severe impairment (total bilirubin >3 to 10 times ULN and any AST): There are no dosage adjustments provided in the manufacturer’s labeling (has not been studied); avoid use.

Active liver or biliary tract disease (including increased alkaline phosphatase): Avoid pexidartinib use.

Withhold pexidartinib and monitor LFTs twice weekly. If an alternate cause for increased bilirubin is confirmed and bilirubin is < ULN within 4 weeks, resume pexidartinib at a reduced dose. If bilirubin is not < ULN within 4 weeks, permanently discontinue pexidartinib.

Total bilirubin ≥2 times ULN or direct bilirubin >1.5 times ULN

Permanently discontinue pexidartinib. Monitor LFTs twice weekly until bilirubin is ≤ ULN.

Dosing: Adjustment for Toxicity: Adult

Severe or intolerable adverse reactions or lab abnormalities (other than hepatic): Withhold pexidartinib until improvement or resolution. Resume pexidartinib at a reduced dose upon improvement or resolution.

Dermatologic: Hair discoloration (67%), pruritus (18%), skin rash (28%)

Endocrine & metabolic: Decreased serum phosphate (25%), hypercholesterolemia (44%), increased lactate dehydrogenase (92%)

Gastrointestinal: Constipation (12%), decreased appetite (16%), dysgeusia (26%; including ageusia), vomiting (20%)

Hematologic & oncologic: Decreased hemoglobin (30%), decreased neutrophils (44%; grade ≥3: 3%), decreased platelet count (15%), lymphocytopenia (38%; grade ≥3: 2%)

Contraindications

There are no contraindications listed in the manufacturer's labeling.

Warnings/Precautions

Concerns related to adverse effects:

• Hepatotoxicity: Pexidartinib can cause serious and potentially fatal liver injury. Hepatotoxicity, including liver failure and life-threatening vanishing bile duct syndrome (VBDS), ductopenia, and symptomatic cholestasis (including severe pruritus), may occur despite monitoring and prompt drug discontinuation. Two irreversible cases of cholestatic livery injury occurred in clinical trials (which were fatal or required a liver transplantation). In the REMS program, 32 out of 609 patients developed hepatotoxicity within the first 71 days of the first dose (10 patients required hospitalization, 2 developed VBDS); 16 of the 32 patients had not fully recovered at the time of analysis, including 6 patients followed for ≥6 months after discontinuation of treatment. The occurrence of pexidartinib-related cholestatic hepatoxicity cannot be predicted, and the mechanism of hepatotoxicity is unknown. It is not known if liver injury occurs in the absence of transaminase elevations. A small percentage of patients in a clinical study who received pexidartinib developed signs of serious liver injury (ALT or AST ≥3 × ULN with total bilirubin ≥2 × ULN). In these patients, peak ALT ranged from 6 to 9 × ULN, peak total bilirubin ranged from 2.5 to 15 × ULN, and alkaline phosphatase was ≥2 × ULN. ALT, AST, and total bilirubin improved to <2 × ULN at 1 to 7 months after discontinuing pexidartinib. Avoid pexidartinib in patients with preexisting increased serum transaminases, total bilirubin or direct bilirubin (> ULN), or active liver or biliary tract disease (including increased alkaline phosphatase). Avoid concurrent administration of other hepatotoxic medications in patients with elevated transaminases, elevated total or direct bilirubin (> ULN), or active liver or biliary tract disease. Refer patients to a hepatologist if liver tests do not return to normal.

• Photosensitivity: Patients should avoid prolonged sun exposure during treatment; advise patients to wear protective clothing and use broad-spectrum UVA/UVB sunscreen.

Other warnings/precautions

• High-fat meal risk: Administering pexidartinib with a high-fat meal increases pexidartinib concentrations, which could increase the incidence and severity of adverse reactions (including hepatotoxicity). Instruct patients to take pexidartinib with a low-fat meal (~11 to 14 g of total fat) and to avoid taking pexidartinib with a high-fat meal (~55 to 65 g of total fat). Consider a referral to a dietitian if needed.

Administration: Adult

Oral: Administer with a low-fat meal (~11 to 14 g total fat). Administration with a high-fat meal (~55 to 65 g total fat) increases pexidartinib concentrations and may increase the risk for adverse reactions, including hepatotoxicity. Swallow capsules whole; do not open, break, or chew. If a dose is vomited, administer the next dose at its scheduled time.

Hazardous Drugs Handling Considerations

This medication is not on the NIOSH (2024) list; however, it may meet the criteria for a hazardous drug. Pexidartinib may cause teratogenicity, reproductive toxicity, and has a structural or toxicity profile similar to existing hazardous agents.

Use appropriate precautions for receiving, handling, storage, preparation, dispensing, transporting, administration, and disposal. Follow NIOSH and USP 800 recommendations and institution-specific policies/procedures for appropriate containment strategy (NIOSH 2023; NIOSH 2024; USP-NF 2020).

Tenosynovial giant cell tumor, symptomatic: Treatment of symptomatic tenosynovial giant cell tumor (TGCT) associated with severe morbidity or functional limitations and not amenable to improvement with surgery in adults.

Food Interactions

Administration of pexidartinib 400 mg with a high-fat meal (800 to 1,000 calories with fat as 50% of caloric content [~55 to 65 g of total fat]) increased the Cmax and AUC by 100% and delayed the time to peak by 2.5 hours; administration of pexidartinib 400 mg with a low-fat meal (387 calories with fat ~25% of caloric intake [~11 to 14 g of total fat]) increased the Cmax and AUC by 56% and 59%, respectively, and delayed the time to peak by 1.5 hours. Grapefruit juice may increase pexidartinib serum concentrations. Management: Administer capsules with a low-fat meal (~11 to 14 g of total fat). Avoid grapefruit juice.

Reproductive Considerations

Evaluate pregnancy status prior to treatment initiation in patients who could become pregnant. Patients who could become pregnant should use effective nonhormonal contraception during treatment and for 1 month after the last pexidartinib dose. Patients with partners who could become pregnant should use effective contraception during treatment and for 1 week after the last pexidartinib dose.

Pregnancy Considerations

Based on the mechanism of action and data from animal reproduction studies, in utero exposure to pexidartinib may cause fetal harm.

Breastfeeding Considerations

It is not known if pexidartinib is present in breast milk.

Due to the potential for serious adverse reactions in the breastfed infant, breastfeeding is not recommended by the manufacturer during treatment and for at least 1 week after the last pexidartinib dose.

Dietary Considerations

Avoid grapefruit juice.

Monitoring Parameters

Monitor LFTs (including AST, ALT, total bilirubin, direct bilirubin, alkaline phosphatase, and gamma-glutamyl transferase [GGT]) prior to pexidartinib initiation, weekly for the first 8 weeks, every 2 weeks for the next month, and then every 3 months thereafter; monitor more frequently in patients with hepatotoxicity; following rechallenge, monitor LFTs weekly for the first month. Monitor kidney function. Evaluate pregnancy status prior to treatment initiation (in patients who could become pregnant). Monitor for signs/symptoms of liver injury. Monitor adherence.

The American Society of Clinical Oncology hepatitis B virus (HBV) screening and management provisional clinical opinion (ASCO [Hwang 2020]) recommends HBV screening with hepatitis B surface antigen, hepatitis B core antibody, total Ig or IgG, and antibody to hepatitis B surface antigen prior to beginning (or at the beginning of) systemic anticancer therapy; do not delay treatment for screening/results. Detection of chronic or past HBV infection requires a risk assessment to determine antiviral prophylaxis requirements, monitoring, and follow-up.

Mechanism of Action

Pexidartinib is a tyrosine kinase inhibitor with selective and strong inhibitory activity against the colony-stimulating factor 1 receptor (CSF1R); pexidartinib also inhibits KIT and FMS-like tyrosine kinase 3 (FLT3)-internal tandem duplication (Tap 2019). Overexpression of the CSF1R ligand promotes cell proliferation and accumulation in the synovium. Pexidartinib inhibits proliferation of a CSF1R dependent cell line.

Pharmacokinetic note: According to the manufacturer, there is no clinically significant difference in relative exposure between administration of 250 mg twice daily with a low-fat meal and 400 mg twice daily on an empty stomach.

Protein binding: >99%; to albumin and alpha-1 acid glycoprotein.

Metabolism: Oxidation via CYP3A4 and glucuronidation via UGT1A4 (glucuronidation forms inactive N-glucuronide metabolite).

Half-life elimination: 26.6 hours.

Time to peak: 2.5 hours.

Excretion: Feces (65%; 44% as unchanged drug); Urine (27% as metabolites [≥10% as N-glucuronide]); Clearance: 5.1 L/hour.

Pharmacokinetics: Additional Considerations (Adult Data Unless Noted)

Altered kidney function: Mild, moderate, and severe impairment (CrCl 15 to 89 mL/minute) increased pexidartinib exposure by ~30%, compared to normal kidney function (CrCl ≥90 mL/minute).

Hepatic function impairment: Moderate impairment (1.5 to 3 times ULN [not due to Gilbert syndrome] and any AST) increased pexidartinib exposure by 43% compared to normal hepatic function (total bilirubin and AST ≤ ULN).
