Total pancreatectomy MRCP
A 74-year-old previously healthy woman presented with abdominal pain, weight loss, and 6 months of worsening diarrhea. She was diagnosed with diabetes and exocrine insufficiency, and a noncontrast CTAP demonstrated a dilated pancreatic duct. A follow-up MRCP (pictured here) demonstrated a uniformly dilated main pancreatic duct, with multiple less dilated side branch lesions. The intra- and extrahepatic ducts were also dilated. The patient underwent a total pancreatectomy without issue. Pathology demonstrated areas of low- and high-grade dysplasia throughout the main pancreatic duct, but no carcinoma was found in the pancreas or regional lymph nodes.
