Noncontrast CT in transverse axial (A) and coronal (B) planes show fluid and focal osteolysis (*) in right mastoid consistent with coalescent mastoiditis. Note thinning of mastoid wall (arrow) abutting junction of transverse-sigmoid venous sinus.
Trace diffusion-weighted MR images (C, D) show hyperintense signal in mastoid and middle ear (circled).
Pre- (E) and post-contrast (F) MR imaging shows enhancement of coalescent otomastoiditis (dashed arrow) and filling-defect (short arrow) in right transverse-sigmoid sinus junction.
Coronal post-contrast MR imaging (G) shows filling-defect (thick arrow) consistent with thrombus in proximal right transverse sinus.
MR venography (H) shows absent flow in left transverse and sigmoid sinuses.
