Surveillance for individuals with heritable TP53-related cancer syndrome (hTP53rc)

Surveillance for individuals with heritable TP53-related cancer syndrome (hTP53rc)
Age Recommended examinations
Children and adolescents
  • Clinical examination every 6 months until age 17 years, then annually*
  • Abdominal ultrasound every 6 months until age 18 years
  • Whole body MRI without contrast annually
  • Brain MRI (first scan with contrast, remainder without contrast) annuallyΔ
Adults
  • Clinical examination annually
  • Whole body MRI annually
  • Breast MRI annually for females age 20 to 65 years
  • Brain MRI annually until age 50 years
  • Colonoscopy every 5 years starting at age 18 years
Refer to UpToDate for additional information about heritable TP53-related cancer syndrome (hTP53rc), Li-Fraumeni syndrome (LFS), and TP53 genetic testing.

hTP53rc: heritable TP53-related cancer syndrome; MRI: magnetic resonance imaging; US: United States.

* Clinical examination includes a standard physical examination with attention to blood pressure and the skin in any radiation fields. In children, signs of virilism or early puberty are also evaluated. This is started at birth and continued every 6 months to age 17 years, and annually thereafter.

¶ Urine steroids are measured if abdominal ultrasound does not allow adequate imaging of the adrenal glands.

Δ Brain and whole body MRI are done without gadolinium contrast, with the exception of the first brain MRI, which is done with contrast. In children, brain MRI and whole body MRI are alternated every 6 months so that the brain is imaged every 6 months.

◊ Colonoscopy is only used in those with a positive personal history of abdominal irradiation or a positive family history of colorectal cancer suggestive of increased risk. In the US, colonoscopies are routinely done in all adults.
