* Dosing will be different if a proton pump inhibitor other than omeprazole or esomeprazole is used.

¶ High-dose amoxicillin is used as part of a specific regimen (with a proton pump inhibitor and metronidazole) for patients with multiple antibiotic resistance.

Δ When amoxicillin or metronidazole is used for rescue therapy (ie, if a prior attempt at treatment was unsuccessful), the UpToDate authors typically recommend higher doses, eg, amoxicillin 75 mg/kg/day divided 3 or 4 times daily or metronidazole 50 mg/kg/day divided 3 times daily. The increased frequency of dosing helps to avoid low trough levels.

◊ Clarithromycin is available as a reconstituted oral suspension. If the oral suspension is used, the total daily dose can be administered in 2 equally divided doses (ie, for patients 25 to 34 kg, administer 375 mg twice daily).

§ Metronidazole oral suspension (50 mg/mL) can be compounded by a pharmacist using oral tablets or a compounding kit. If the oral suspension is used, the total daily dose can be administered in 2 equally divided doses (ie, for patients 25 to 34 kg, administer 375 mg twice daily).
