CT and MRI of 32-year-old patient with seizures from neurocysticercosis
32-year-old patient with seizures from colloidal vesicular neurocysticercosis. A noncontrast head CT (A) shows multiple, small cortically based cysts. On transverse axial FLAIR MRI (B, D) there are multiple cysts, including one in the left frontal operculum that is surrounded by vasogenic edema (arrow). A magnified FLAIR image (C) shows a thin-walled larval cyst (hexcanth) in left frontal lobe that contains an invaginated scolex or larval head (dashed arrow). A post-contrast T1-weighted MRI (E) demonstrates incomplete-rim enhancement around the frontal opercular cyst. Contrast enhancement and edema are secondary to an inflammatory response of the host to dying larva.
