Concerns related to adverse effects:
• Hepatotoxicity: Sotorasib may cause hepatotoxicity and increased ALT or AST, which may result in drug-induced liver injury and hepatitis. In a clinical trial of patients with non–small cell lung cancer (NSCLC) treated with single-agent sotorasib, hepatotoxicity occurred in approximately one-fourth of treated patients, including grade ≥3 events. Among patients receiving single-agent sotorasib with hepatotoxicity requiring dose modification, two-thirds required corticosteroids for management of hepatotoxicity. The median time to first onset of elevated ALT and/or AST was 6.3 weeks (range: 0.4 to 42 weeks). Drug-induced liver injury occurred in a small percentage of patients. Patients receiving immunotherapy ≤3 months prior to starting sotorasib (as a single agent) demonstrated a higher incidence of hepatotoxicity than those that received immunotherapy >3 months prior to starting sotorasib and those who never received immunotherapy. Most hepatotoxicity events improved or resolved with dose modification of sotorasib, with or without corticosteroid treatment. Hepatotoxicity also occurred when sotorasib was used in combination with panitumumab for the management of colorectal cancer (including grade 3 events); the median time to first onset of elevated ALT or AST was 10 weeks (range: 2 to 22 weeks) and the median time to first onset of hyper bilirubinemia was 12 weeks (range: up to 29 weeks). Among patients receiving sotorasib in combination with panitumumab with hepatotoxicity requiring dose modification, one-fifth required corticosteroids for management of hepatotoxicity.
• Pulmonary toxicity: Interstitial lung disease (ILD)/pneumonitis may occur and could be fatal. In clinical trials, ILD/pneumonitis occurred rarely, including grade 3 or 4 cases, and a fatality. The median time to first onset of ILD/pneumonitis with sotorasib (as a single agent) for NSCLC was 8.6 weeks (range: 2.1 to 36.7 weeks).

Oral: Administer at the same time each day with or without food. Swallow whole; do not chew, crush, or split tablets.
When used in combination with panitumumab, administer the first sotorasib dose prior to the first panitumumab infusion.
If unable to swallow tablets, an oral mixture may be prepared. Disperse tablets in 120 mL (4 oz) of noncarbonated, room temperature water (do not crush tablets); do not use other liquids. Stir or swirl the cup for ~3 minutes until tablets are dispersed into small pieces; the tablets will not completely dissolve (the mixture may appear pale yellow to bright yellow). Drink immediately or within 2 hours of dispersion; do not chew tablet pieces. Rinse the container with an additional 120 mL of noncarbonated water and swallow. If the mixture is not administered immediately, stir again to ensure that the tablets are dispersed.

In animal reproduction studies, maternal toxicity was observed in rats and rabbits administered sotorasib in doses equivalent to ~4.6 times and ~2.6 times the human dose, respectively (based on AUC). Embryo-fetal adverse events were not observed in the rat studies at this dose level; decreased fetal body weight and a reduced number of ossified metacarpals were observed in the fetuses of rabbits.
It is not known if sotorasib is present in breast milk.
Due to the potential for serious adverse reactions in the breastfed infant, breastfeeding is not recommended by the manufacturer during therapy and for 1 week after the last dose of sotorasib.
KRAS G12C mutation status (for non–small cell lung cancer, in tumor or plasma specimens [if no mutation is detected in a plasma specimen, test tumor tissue]; for metastatic colorectal cancer, in tumor specimens). Monitor LFTs (ALT, AST, alkaline phosphatase, and total bilirubin) prior to sotorasib initiation, every 3 weeks for the first 3 months, then once a month or as clinically indicated (monitor more frequently in patients who develop transaminase and/or bilirubin elevations or in patients with altered liver function). Monitor for new or worsening signs/symptoms of interstitial lung disease/pneumonitis (eg, dyspnea, cough, fever). Monitor adherence.
The American Society of Clinical Oncology hepatitis B virus (HBV) screening and management provisional clinical opinion (ASCO [Hwang 2020]) recommends HBV screening with hepatitis B surface antigen, hepatitis B core antibody, total Ig or IgG, and antibody to hepatitis B surface antigen prior to beginning (or at the beginning of) systemic anticancer therapy; do not delay treatment for screening/results. Detection of chronic or past HBV infection requires a risk assessment to determine antiviral prophylaxis requirements, monitoring, and follow-up.
Sotorasib inhibits KRAS G12C, which is a tumor-restricted, mutant-oncogenic form of KRAS. Sotorasib forms a covalent irreversible bond with the unique cysteine of KRAS G12C, locking the protein in an inactive state that prevents downstream signaling; wild-type KRAS is not affected. Sotorasib displays minimal off-target activity; it blocked KRAS signaling, inhibited cell growth, and promoted apoptosis in KRAS G12C cell lines only. In KRAS G12C animal models, sotorasib treatment resulted in tumor regression and prolonged survival (and was associated with antitumor immunity). In the KRAS G12C-mutant metastatic colorectal cancer setting, epidermal growth factor receptor (EGFR) activation is a resistance mechanism for KRAS G12C inhibition. In animal colorectal tumor models, the combination of sotorasib and panitumumab demonstrated increased antitumor activity compared to either agent alone.
