Syndesmosis injury with wide medial clear space

The initial AP radiograph taken of a high school American football player showed a fibula fracture and syndesmosis rupture initially treated with ORIF and repair of the deltoid ligament, but without fixation of the syndesmosis. Eight weeks after the injury, the repeat AP radiograph shown above demonstrated chronic syndesmosis disruption, wide medial clear space, and incomplete healing of fibular shaft.
