This algorithm describes the general approach to preparing a patient with chronic respiratory failure due to neuromuscular or chest wall disease for a trial of noninvasive ventilation (NIV) to increase the chances of success. Refer to UpToDate content on selecting a mode of ventilation and titration modules for NIV in this population.

* Selection of the mask is the most important step and should be individualized. Selecting an ideal mask is dependent upon factors including patient comfort, mask availability, facial contour, and the patients' or caregivers' ability to remove the mask quickly in the event of an emergency (eg, power failure or vomiting into the mask, which increases the risk of aspiration or asphyxiation).

¶ Choosing among these options is at the discretion of the individual facilitating the trial. Distraction techniques may be successful in anxious patients; these involve wearing the mask under distraction such as watching TV or reading.
