Management of keloids

* Small, moderate size single keloids are more likely to respond to conservative therapies than large keloids.

¶ Large keloids are less likely to respond to conservative therapies and may require a more aggressive approach with surgery and postoperative adjuvant therapy.

Δ Multimodal nonsurgical therapy includes 1 or more of the following, used concurrently or in sequence:
  • Corticosteroid tape/plaster
  • Intralesional injections of corticosteroids
  • Intralesional injections of fluorouracil or bleomycin
  • Compression therapy
  • Cryotherapy
  • Laser therapy

◊ Postoperative adjuvant therapies include:

  • Radiation therapy (preferred, adult patients only)
  • Intralesional injections of corticosteroids
  • Intralesional injections of fluorouracil or bleomycin

§ Conservative therapies include:

  • Compression therapy
  • Gel sheets
  • Tape fixation
  • Laser therapy
