Factors favoring surveillance Factors favoring anticoagulation
  • No symptoms (or minor symptoms)
  • High-risk features of proximal extension are absent (refer to the "Factors favoring anticoagulation" column)
  • Minor thrombosis in the muscular veins
  • A negative D-dimer level (ie, <500 ng/mL)
  • Nondiagnostic ultrasonography results
  • High risk of or contraindications to bleeding
  • Patient preference
  • Symptoms (other than minor ones)
  • Thrombus extension into or close to (eg, 1 to 2 cm) the proximal popliteal vein
  • Extensive thrombosis involving multiple veins (eg, >5 cm in length, >7 mm in diameter)
  • D-dimer ≥500 ng/mL
  • Unprovoked DVT
  • Persistent/irreversible risk factors, such as active cancer or prolonged immobility
  • Previous DVT or pulmonary embolism
  • Inpatient status
  • COVID-19
  • Patient preference
