Acute treatment of tension-type headache in adults

Acute treatment of tension-type headache in adults
Pharmacotherapy for tension-type headache is often self-directed but may be prescribed by a clinician for severe or recurrent cases or in the setting of intolerance or unresponsiveness to initial medications. In general, medications should be administered early after headache onset using an effective single dose to optimize efficacy and reduce the risk of medication overuse headache. For patients with an inadequate response within two to four hours of initial dosing, additional evaluation and therapy may be warranted.

TTH: tension-type headache; IV: intravenous; IM: intramuscular; NSAIDs: nonsteroidal antiinflammatory drugs.

* For patients weighing <50 kg or >64 years old, use a reduced dose of ketorolac (15 mg IV or 30 mg IM once).

¶ Acetaminophen is the preferred option in pregnancy and for patients unable to tolerate NSAIDs or aspirin.

Δ Refer to UpToDate topic for additional details on the differential diagnosis of tension-type headache.

◊ Several medications may be used for preventive treatment of tension-type headache in adults including tricyclic antidepressants and antiseizure medications. Refer to UpToDate topics for additional details.
