This figure summarizes our suggested approach to evaluation of a child with suspected STEC-HUS. Signs and symptoms may include recent diarrhea and multisystem findings (decreased urine output, paleness, bruising, lethargy). Diarrhea may be bloody or non-bloody. Infrequently, diarrhea is absent in STEC-HUS. Absence of diarrhea should not exclude the diagnosis of STEC-HUS. STEC-HUS: hemolytic uremic syndrome due to Shiga toxin producing E. coli; CBC: complete blood count; BUN: blood urea nitrogen; STEC: Shiga toxin-producing E. coli; HUS: hemolytic uremic syndrome; PCR: polymerase chain reaction; TTP: thrombotic thrombocytopenic purpura; DIC: disseminated intravascular coagulation; PT: prothrombin time; aPTT: activated partial thromboplastin time.

* Although uncommon, if active bleeding is present, coagulation studies (PT, aPTT) are sent.
