A 59-year-old presented with bilateral abducens nerve palsy and multiple facial bone fractures after a motor vehicle collision. (A) At the level of the upper midbrain, 3D-SPACE axial MRI shows hyperintensity in the right cerebral peduncle (arrowhead) consistent with contusion. At the level of the pontomedullary junction (B) and midpons (C), there is a nodular appearance of the avulsed right abducens nerve (circles) compared with normal left abducens nerve (arrows). (D) At the level of the Dorello canal both abducens nerves are normal (arrows).
