Pineal parenchymal tumor obstructing the third ventricle
46-year-old presenting with progressive headaches. Noncontrast CT (A) shows ventriculomegaly involving the lateral and third ventricles. In the pineal region, there is a solid mass (circle) with focal calcifications. Transverse-axial FLAIR MRI (B, C) shows a hyperintense pineal mass (circle) as well as transependymal edema (arrows), indicating hydrocephalus. Precontrast T1-weighted (D) and postcontrast, fat-saturated T1-weighted (E) MRI shows a homogeneously-enhancing pineal mass (circle) obstructing the posterior third ventricle, just cephalad to the cerebral aqueduct. Histopathology confirmed a pineal parenchymal tumor, not otherwise specified.
