Indications for Helicobacter pylori testing
Established indications*
  • Individuals with dyspepsia (including those undergoing endoscopy)
  • Individuals with peptic ulcer disease or past history of peptic ulcer
  • Individuals with current or prior gastric cancer, including early gastric cancer resection
  • Individuals with atrophic gastritis, gastric intestinal metaplasia, or gastric dysplasia
  • Individuals with autoimmune gastritis
  • Individuals with gastric marginal zone lymphoma of MALT
  • Individuals with hyperplastic gastric polyps or gastric adenomas
  • Individuals with a first-degree relative with gastric cancer
Supported by limited evidence
  • Prior to long-term treatment with NSAIDs and/or low-dose aspirin
  • Individuals with unexplained iron deficiency anemia
  • Individuals with immune thrombocytopenia
  • Individuals who immigrate from a country with a high prevalence of H. pylori and/or incidence of gastric cancer
  • Adult household members of individuals diagnosed with H. pylori infection

H. pylori: Helicobacter pylori; MALT: mucosa-associated lymphoid tissue; NSAIDs: nonsteroidal anti-inflammatory drugs.

* H. pylori eradication treatment should be offered to all individuals who test positive on nonserologic testing (indicating active infection) unless there are contraindications (eg, pregnancy, limited life expectancy). Refer to UpToDate content on the diagnosis of H. pylori for details on test selection and timing of testing.
