Initial management of peritoneal dialysis-associated peritonitis without catheter exit-site or tunnel infection

Initial management of peritoneal dialysis-associated peritonitis without catheter exit-site or tunnel infection
Patients with peritonitis and a catheter exit-site or tunnel infection require catheter removal in addition to antibiotics. Examination of the catheter exit site and area over the catheter pathway is part of the evaluation of patients with peritonitis.Δ

IP: intraperitoneal; IV: intravenous.

* Patients with sepsis are more likely to have secondary peritonitis due to underlying pathology of the gastrointestinal tract. For evaluation and management of suspected secondary peritonitis, refer to UpToDate content on the clinical manifestations and diagnosis of peritonitis in peritoneal dialysis and UpToDate content on the microbiology and therapy of peritonitis in peritoneal dialysis.

¶ The initial broad-spectrum empiric antibiotic regimen should cover gram-positive and gram-negative organisms and should be narrowed after a specific organism is identified. For details, refer to UpToDate content on the microbiology and therapy of peritonitis in peritoneal dialysis.

Δ Refer to UpToDate content on peritoneal catheter exit-site and tunnel infections in peritoneal dialysis, and UpToDate content on the clinical manifestations and diagnosis of peritonitis in peritoneal dialysis.
