Physical frailty phenotype

Physical frailty phenotype
Characteristic Measurement Meets criteria if:
Shrinking
  • Unintentional weight loss
  • Sarcopenia (loss of muscle mass)
Weight changes

>10 lbs or 5% body weight unintentional weight loss in the past year

or

BMI <18.5 kg/m2
Weakness Grip strength
(requires hand dynamometer)
Lowest 20% (by sex, BMI)
Males: Females:
BMI Cutoff for grip strength (Kg) criterion for frailty BMI Cutoff for grip strength (Kg) criterion for frailty
≤24 ≤29 ≤23 ≤17

26.1 to 28 ≤30 26.1 to 29 ≤18
>28 ≤32 >29 ≤21
Poor endurance
  • Exhaustion
Self-report of exhaustion Subjective patient report to following:
  • I felt that everything I did was an effort
    • (Answers: yes, no, refused, don't know)
  • If yes, how often in the last week did you feel this way?
    • 3 - Most of the time
    • 2 - Moderate amount of time (3 to 4 days)
    • 1 - Some of the time (1 to 2 days)
    • 0 - Rarely/none of the time (<1 day)
  • I could not get going

Answers of 2 or 3 to either question meet criteria for exhaustion

Slowness Gait speed per 15-foot distance Slowest 20% (by sex, height)
Males: Females:
≤7 seconds for height ≤173 cm ≤7 seconds for height ≤159 cm
≤6 seconds for height >173 cm ≤6 seconds for height >159 cm
Low activity Based on short version of the Minnesota Leisure Time Activity Questionnaire to determine Kcals per week expended Lowest 20% (by sex)
  • Males: <383 Kcals/week expended
  • Females: <270 Kcals/week expended

Frailty: ≥3 out of 5 characteristics present.

Intermediate or prefrail: 1 to 2 out of 5 characteristics present.

Robust: 0 out of 5 characteristics present.
BMI: body mass index.
