Primary cutaneous anaplastic large cell lymphoma: Initial management

Primary cutaneous anaplastic large cell lymphoma: Initial management
This algorithm describes the initial management of PC-ALCL in patients with no evidence of visceral disease (ie, no bone marrow or organ involvement). Refer to related UpToDate material for details of diagnosis, staging, symptom management, and acceptable alternative approaches. Management of systemic ALCL is discussed separately.

ALCL: anaplastic large cell lymphoma; PC-ALCL: primary cutaneous anaplastic large cell lymphoma; RT: radiation therapy.

* Imaging should be performed to evaluate clinical lymphadenopathy or unexplained organ enlargement or dysfunction. Biopsy should be performed for abnormal imaging to exclude involvement by ALCL. Unexplained hematologic abnormalities should be investigated with a bone marrow aspirate/biopsy. If widespread nodal or visceral disease is present, we treat like systemic ALCL, as described in related content on initial treatment for systemic ALCL.

¶ Refer to related UpToDate content for alternative systemic agents in patients who are not candidates for brentuximab vedotin (eg, pre-existent peripheral neuropathy).
