Vulvar cancer: Extent of surgical excision

Management of vulvar cancer includes both surgical excision and evaluation of lymph nodes. This algorithm describes how to determine the extent of the surgical excision. The approach to evaluation of the lymph nodes is described in a separate algorithm.

Management of patients in whom surgery is not feasible (eg, locally advanced disease; tumors involving critical midline structures; medically frail patients; distant metastatic disease) is discussed in related UpToDate topics.
* Tumor size, depth of invasion, and local extension are determined mostly by physical examination and vulvar biopsy. Imaging can also be used, particularly for evaluation of lymph nodes. This is described in detail separately.
