Treatment for children with enthesitis related arthritis

Treatment for children with enthesitis related arthritis
To be used with UpToDate content on treatment of ERA. For information on drug selection, administration, and adverse effects, refer to UpToDate content on the treatment of spondyloarthrtis.

ERA: enthesitis related arthritis; IA: intraarticular; IL: interleukin; NSAID: nonsteroidal antiinflammatory drug; TNF: tumor necrosis factor.

* In children who do not tolerate oral NSAIDs, topical diclofenac may be an alternative for treating small-joint ERA.

¶ In patients with mild arthritis that is not interfering with daily activities, it may be reasonable to defer IA glucocorticoids, especially if sedation would be required.

Δ For moderate to severe peripheral arthritis, sacroiliitis, and/or enthesitis, patients may benefit from the addition of a short course of oral glucocorticoids as bridging therapy while starting systemic immunosuppression (eg, 10 to 20 mg/day of prednisone for no more than 2 weeks).
