Chronic secondary mitral regurgitation: An approach to referral for intervention
The algorithm presents an approach to referral for MV intervention for patients with secondary MR. Secondary MR is defined as MR caused by left ventricular and/or left atrial changes. A multidisciplinary heart team approach is recommended for the evaluation and care of potential candidates for MV intervention. Refer to UpToDate content on secondary MR, which includes a description of the evidence supporting this approach.

LVEF: left ventricular ejection fraction; MR: mitral regurgitation; MV: mitral valve; TEER: transcatheter edge-to-edge repair.

* For most patients undergoing MV surgery for secondary MR, MV replacement with chordal sparing is preferred to MV repair.

¶ Optimized heart failure management includes evidence-based pharmacologic therapy and cardiac resynchronization therapy, as indicated.

Δ Contraindications to TEER include known hypersensitivity to clip components (nickel/titanium, cobalt, chromium, polyester), contrast sensitivity, active endocarditis on the MV, or rheumatic MV disease.
