Role of surgery for acute aortic regurgitation
This algorithm describes a general approach to identifying patients with acute AR who may require surgical intervention (including valve replacement or repair). In patients with indications for emergency or urgent aortic valve surgery, medical therapy is only temporizing. Refer to UpToDate content on the management of acute AR.

AR: aortic regurgitation.

* Signs of hemodynamic instability may include hypotension, pulmonary edema, and/or signs of inadequate perfusion.

¶ In patients with acute AR caused by aortic dissection, in some cases, graft replacement of the ascending aorta may restore normal aortic valve function without the need for an aortic valve procedure.

Δ Indications for valve surgery in the setting of endocarditis may include paravalvular abscess, infection with a difficult-to-treat pathogen (eg, fungus), or a large mobile vegetation.
