Approach to determining irritable bowel syndrome (IBS) subtype

This algorithm summarizes the approach to determining IBS subtype in patients diagnosed with IBS. The IBS subtype guides the selection of treatment. Because IBS subtypes are dynamic, we confirm IBS subtype regularly, especially in patients with refractory or worsening symptoms. Please see related UpToDate content for additional detail.

* The patient should be evaluated while off medications that may impact bowel habits, including medications for IBS.

¶ The assignment of a stool as "abnormal" is based on the patient's perception, compared with their usual stool pattern.

Δ Although IBS-U is technically an established subtype, when determining treatment for IBS-U, we reclassify based on our best estimation of the predominant bowel habit and follow the associated treatment approach.
