Clinical evaluation for defecatory dysfunction

Clinical evaluation for defecatory dysfunction
Suggestive symptoms
  • Excessive straining
  • Prolonged defecation time
  • Feelings of incomplete evacuation
  • Use of digital maneuvers to defecate
Suggestive rectal exam findings
  • High anal resting pressure
  • Impaired push effort
  • Paradoxical contraction of the anal sphincter and puborectalis muscles during simulated evacuation

Defecatory dysfunction refers to impaired rectal evacuation and is usually due to discoordination of muscles in the pelvic floor (ie, dyssynergic dysfunction).

This table outlines clinical clues suggestive of defecatory dysfunction in patients being evaluated for chronic constipation. Diagnosis can be confirmed with anorectal manometry and balloon expulsion testing. Treatment is usually with biofeedback therapy.
