How do I keep track of my weight? —
You can use a chart to help you keep track (form 1).
Every time you weigh yourself, write down:
●The date and time
●Your weight
●Important details you want to remember – For example, you can note what you ate, if you took your medicine, and how you were feeling.
Your doctor or nurse can tell you:
●How and when to weigh yourself
●When to take an action, such as calling the doctor, based on changes in your weight
