The diurnal pattern of glucose values, with hyperglycemia during the day and improving glucose values at bedtime and before breakfast, suggests that hospitalization is not necessary.

While the use of an intravenous insulin infusion is often appropriate in a hospitalized patient with uncontrolled diabetes, it is desirable to avoid hospitalization if possible. The hospital is an artificial environment. The changes in diet and physical activity associated with hospitalization alter the patient's insulin requirement. The final adjustment in the insulin regimen is usually made when the patient returns to the home environment after discharge.
