This figure illustrates the uncommon but high-risk anatomic variation where the recurrent laryngeal nerve (RLN) courses lateral to the tubercle of Zuckerkandl, which is often enlarged. The nerve is encountered much earlier in the dissection required for thyroidectomy and is at risk of being misidentified as a vascular structure.
