Lateral radiograph of the cervical spine shows a fracture through the C2 vertebral body (arrow). The extension teardrop fracture occurs when abrupt neck extension causes the anterior longitudinal ligament to pull the anteroinferior portion of the vertebral body away from the remainder producing a triangular-shaped fracture fragment (teardrop).
